This page contains links to the forms that are routinely used by the national officers, board members, and committee chairs. Below you should find all of the forms you will need.
If you are not a member of the national board of directors, or an approved national committee chairperson, please do not attempt to access or use these forms. They will not be valid.

*FOR BEST RESULTS, MAKE SURE YOU HAVE THE MOST RECENT VERSION OF ACROBATE READER DC DOWNLOADED ON YOUR COMPUTER. THIS IS A FREE PROGRAM, AND IS UNIVERSALLY TRUSTED. THIS VERSION WILL ALLOW YOU TO FILL OUT THE FORM, AND SAVE IT WITH YOUR INFORMATION. CLICK HERE TO GO TO THE DOWNLOAD PAGE. NOTE: Trying to fill out forms on a phone or some tablets does not work without the correct app. The apps that are used by these devices do not always play well with fillable documents. You will have better results if you are using a computer or laptop.
PDF FORMS: when the form opens, save it (download it) to your hard drive before attempting to fill it out. They often do not respond correctly until saved.
PDF fillable forms: These forms will not function correctly or be savable once filled in if you do not have a proper PDF reader on your computer/laptop. The (free) Adobe Reader DC is the easiest to use. Without this, you must print them, fill them out and sign them (if required) and scan them back into your computer, in order to save or send them. Be sure when you scan them, you set your printer to save it as a 'Document' file and NOT as a 'jpg'. If you are well versed in Microsoft Word, there are forms at the bottom of this page that you can fill out and save, without having to print and scan.

MICROSOFT WORD FILLABLE FORMS
These forms are in WORD (docx) format and are fillable, (using tables). The easiest way to use these forms is to 'download' or 'save' them to your computer giving them a slightly different name (such as adding your initials, or a number to the end of the file name). Once opened, click your curser into the space you want to type in, rather than trying to use the 'tab' key as you do in the PDF forms. Using the tab key will result in having to tab through every space, including the title spaces. Depending on your browser (Internet Explorer, FireFox, Chrome, AOL, etc) it should either automatically download to your harddrive, or it may open a window asking if you want to 'open' or 'save'. You will want to save it. NOTE: PC users will probably have to click on the yellow line in the upper portion of the window that the document opens in, where it says "Enable". That should only happen the first time you use the form.
There is no way to predict how these forms will open or download, on your computer, as different people use different browsers and operating systems, and different versions (older, newer) of Microsoft Word. If they do not look right or behave correctly when you open them, you may need to use the PDF versions of these forms.
